PEDIATRICS

The Department of Pediatrics is an integral component of The University Institute for Children's Health, the multidisciplinary, cross-departmental pediatric program of University Hospital and the New Jersey Medical School. Attentive to community, national and international health issues, the Institute offers family-centered care to children from birth through adolescence with a full array of ambulatory and inpatient services. Programs are provided for the full range of health care needs, from the well child to children with the most difficult and life-threatening illnesses.

The Department of Pediatrics provides leadership and ensures continuity of care for pediatric patients through its diversified faculty, numerous specialty and sub-specialty services and commitment to excellence in primary care.

The department is supported by an extensive infrastructure of on-site and off-campus ambulatory care programs as well as hospital-based emergency and inpatient services. A dedicated pediatric emergency room, a 11-bed pediatric critical care unit, and a 21- bassinet neonatal intensive care unit provide the most technologically advanced care to the critically ill patient. The hospital offers a 32-bed general pediatric unit and an extensive Child Life Program is maintained to support hospitalized children in all inpatient units through age/developmentally appropriate play activities.

Pediatrics - Adolescent and Young Adult Medicine

The Division of Adolescent and Young Adult Medicine (DAYAM) provides comprehensive primary medical and psychosocial care of adolescents and young adults from the ages of 12 to 25. DAYAM's staff of adolescent specialists also address specialized health and behavioral issues such as adolescent gynecology, substance abuse, physical and sexual abuse, HIV/AIDS, sexually transmitted diseases, eating disorders, and delinquency/conduct disorders.

In addition, DAYAM supports organized programs that focus on adolescent HIV/AIDS prevention and treatment; youth violence prevention and intervention; school dropout prevention; adolescent health promotion; and support for young fathers.

DAYAM's clinical offices are located on the Newark campus. Specialized programs are located in Newark, East Orange, Orange, and Paterson.

Pediatrics - Allergy/Immunology/Infectious and Pulmonary Diseases

James Oleske, MD, MPH, Director

The Division provides ambulatory, in-patient and laboratory services to diagnose and treat acute and chronic infectious diseases as well as pulmonary diseases and allergies. The Division has particular expertise in the management of HIV/AIDS as well as chronic infectious diseases such as tuberculosis. Complete laboratory support is also available for diagnostic skin testing, nasal cytology, spirometry, in-vitro tests for allergic and immunologic disease and HIV.

The Division's active research program studies the transmission of HIV and is well recognized throughout the world. The Francis Xavier Bougnoud Center (FXB) is a program of the Division that is devoted to Pediatric HIV patients and their families. The FXB Center also provides instruction throughout the world to caregivers of HIV infected children while the International HIV Resource Center provides educational programs, resources and instructional materials to health care professionals caring for HIV infected children.

Pediatrics - Ambulatory Pediatrics/Primary Care

Ambulatory Pediatrics/Primary Care offers preventive medicine and provides continuity of care for children from birth to 21 years of age. Emphasis is placed on maintaining a consistent patient-physician relationship. Well-child care includes immunizations, ongoing developmental evaluation, parent education, and a comprehensive approach to healthcare with the participation of social workers, nutritionists and other therapeutic disciplines and diagnostic modalities. A full complement of subspecialties is available, and consultations and referrals are made with ongoing coordination of treatment and follow-up by the primary physician.

Ambulatory/primary care pediatrics is provided at two locations on the Newark campus.

Pediatrics - Cardiology

The Division of Cardiology provides a range of quality cardiac care services, including prenatal diagnosis and monitoring, care of infants, children and adolescents, and management of congenital heart disease patients into adulthood. Inpatient and ancillary support services include echocardiograms and electrocardiograms.

Pediatrics - Child Development

The Child Development program provides diagnostic, medical and therapeutic services to children with developmental and/or behavioral disturbances and children at risk for such disturbances. In cooperation with other clinicians within the University setting, services are coordinated to offer specialized interventions from a multidisciplinary perspective. Collaborating clinicians including pediatric subspecialists, mental health professionals, social workers, nutritionists, and therapists specializing in speech/language, audiology and rehabilitation. The Division also works closely with community agencies and local school districts and provides advocacy to families negotiating needed services and accommodations.

Pediatrics - Critical Care

An 11-bed critical care unit provides high technology intervention for children who have experienced trauma or other life-threatening illnesses. Children in respiratory failure can be managed with conventional or high-frequency oscillation. Additionally, the unit is capable of providing noninvasive ventilation for patients with neuromuscular ventilatory failure, invasive hemodynamic monitoring for those patients requiring cardiovascular support and monitoring and treatment of patients with raised intracranial pressure. The PICU supports the state's only pediatric liver transplant program as well as the Level I trauma center for northern New Jersey. Air and ground transport teams are available for children throughout New Jersey in need of specialized intervention at University Hospital.

Pediatrics - Early Intervention Program

The Early Intervention Program is a State of New Jersey funded program providing education and therapeutic services to infants and toddlers with developmental delays and/or disabilities at no cost to families. Services include developmental evaluation, diagnosis, special education, physical, speech or occupational therapy. Services are available in the Center, the child's home or within community agencies. Parent support groups assist families in coping with the needs of the disabled child.

Pediatrics - Emergency Medicine

Pediatric Emergency Services provides treatment to children and adolescents with acute illnesses and injuries in a dedicated pediatric emergency room. More than 1500 patients are seen each month by pediatricians specially trained in pediatric emergency medicine.

Pediatrics - Endocrinology

The Division of Pediatric Endocrinology treats children and young adults with insulin-dependent diabetes and reproductive, thyroid and growth hormonal disorders. The Division utilizes a comprehensive team approach that helps children better manage and control their illness. The team is comprised of a specially trained physician, diabetes nurse educator, nutritionist and social worker.

Pediatrics - Gastroenterology, Hepatology and Nutrition

The Division provides state-of-the-art services for children with gastrointestinal and liver disorders. Evaluation and management of acute and chronic liver disease and long term followup care for recipients of liver transplants is available for children transplanted at University Hospital or out-of-state centers. Endoscopic procedures including percutaneous gastrostomy placement, liver biopsy, esophageal pH monitoring, and breath testing for carbohydrate intolerance are offered. The division oversees the use of enteral and parenteral nutrition support in hospitalized children and in the home setting.

Research interests include developmental aspects of carbohydrate and bile acid transport, Hepatitis C screening and long term followup in high-risk families, and gastrointestinal aspects of childhood autism.

Pediatrics - Hematology/Oncology

The Division specializes in the diagnosis, evaluation, and treatment of blood diseases occurring from infancy to young adulthood. This includes anemia, platelet and coagulation disorders, blood and lymphatic system malignancies and solid tumors in children. The division supports a multidisciplinary comprehensive sickle cell disease center.

Pediatrics - Human and Molecular Genetics

The Center for Human and Molecular Genetics offers complete medical and maternal-fetal diagnostic services, evaluation and genetic counseling for families of infants and children with multiple congenital malformations, birth defects, and inherited metabolic disorders. In association with the Department of Obstetrics and Gynecology, state-of-the-art prenatal diagnostic services are also provided. The Division, through its outreach programs, delivers genetic services to minority populations throughout New Jersey. Comprehensive genetic laboratory services including cytogenetic analysis, biochemical metabolic testing and DNA (molecular) genetic testing are available. Additionally, the Division provides analysis and care for patients exposed to environmental agents.

Pediatrics - Hypertension Research Center

The Hypertension Research Center is an internationally renowned research center studying essential hypertension and diseases complicated by hypertension, including renal dysfunction and non-insulin dependent diabetes. The Center is funded by The National Institutes of Health and the American Heart Association as well as other foundation grants.

Pediatrics - Lead Poisoning Program

The Lead Poisoning program at University Hospital was one of the first programs in the nation to address the health problems associated with lead contamination. University Hospital is the organizer of a state-wide consortium that provides continuing education for providers of care to lead poisoned individuals.

Dr. Marcus is the only individual in New Jersey board certified in both pediatrics and medical toxicology. University Hospital provides consultative and treatment services for children and adults identified as being lead poisoned.

Pediatrics - Neonatology

The Division is a component of the hospital's State-designated Level III Perinatal Center. The Division specializes in the delivery of sophisticated care to distressed newborns with emphasis on the care of very low birthweight infants. It provides 24-hour transport service for neonatal referrals from outlying hospitals using a specially trained transport team. The staff is available for physician-to-physician consultation regarding problem cases. Attending board-certified neonatologists work closely with referring physicians in planning ongoing care following the immediate crisis, with return of the infant to the local hospital when feasible. Infant progress while at the Center is regularly communicated to the physician.

In addition to low birthweight cases, the division has special expertise in the care of infants with sepsis, blood loss, seizures, respiratory distress syndrome, congenital anomalies, hyperbilirubinemia, surgical conditions, and diagnostic problems. Expanding research programs include fetal development and nutrition in utero and methods of drug withdrawal for infants born to substance abusing mothers.

The Division provides long-term followup care of neonatal intensive care graduates in collaboration with pediatric neurology, child development, social services, and nutrition.

Pediatrics - Nephrology

The Division offers a comprehensive range of diagnostic services and treatment for acute renal failure, hypertension, system and primary kidney disorders, and disorders of acid balance as well as fluid and electrolyte balance. Renal biopsy and renal function tests are available. Continuity of care for the management of pediatric patients with chronic renal failure to the point of dialysis or transplantation is ensured through close affiliation with facilities providing these services.

Pediatrics - Neurology

The Division provides comprehensive diagnostic and treatment services for neurological disorders in infants and children to age 18 (older patients may be seen in special circumstances). These include neonatal disorders, neuromuscular disorders, metabolic disorders, epilepsy, headaches, movement disorders and behavioral disorders, including autism, learning disabilities and school failure.

Special interests of the division members include epilepsy, autism and movement disorders, especially tic disorders/Tourette syndrome/OCD/PANDAS. Division members engage in clinical and/or basic research in their area of interests.

A comprehensive epilepsy program offers all modalities of treatment including standard and experimental medication, ketogenic diet, vagal nerve stimulation and epilepsy surgery (in conjunction with the department of neurosurgery).

The Autism Center, offers neurological evaluation and treatment of patients suspected to have autistic spectrum disorders, or pervasive developmental disorders (PDD).

The Division also includes heads one of the very few pediatric movement disorder clinics in the country, with a focus on tics/OCD/PANDAS, but with interest in a broad range of movement disorders and metabolic diseases.

Division members also participate in specialty clinics in neurogenetics, neurofibromatosis, and neuromuscular diseases.
