Our telecommunications personnel are specially trained
and will assist with arranging the critical care transfer.
However, please be prepared to provide the following
information at the time of the call:
- Patient name, age, weight, diagnosis, reason for
transfer
- Sending and receiving attending physician names
- Patient location at UH
- Phone number for CCTT to contact sending physician
- Bed assignment in receiving facility
When the team arrives, please be ready to provide the
following:
- Written physician order for transfer
- Completed medical necessity form
- Copy of medical records, ECGs, radiographic studies,
lab studies, other diagnostic reports
